Completing a medical questionnaire, requesting treatment, submitting payment, or placing an order does not guarantee that a prescription or particular treatment will be approved.

A healthcare provider may approve, modify, decline, discontinue, or recommend an alternative treatment based on medical information and clinical judgment.

4. Orders, Treatment Plans & Payments

Prices, treatment durations, payment terms, subscription terms, recurring billing arrangements, and applicable fees will be disclosed during the applicable ordering or checkout process.

Treatment interruption may require additional medical review or re-evaluation before treatment can resume.

Refund eligibility may depend on the status of provider review, prescription processing, pharmacy fulfillment, or shipment and is subject to applicable law and DoctorAdam's applicable policies.

11. Patient Responsibilities

When using DoctorAdam Services, you agree to:

• Provide truthful, accurate, and complete information
• Update material changes to your medical information
• Use prescription medications only as directed
• Not share prescription medications with another person
• Contact a healthcare professional if you experience concerning symptoms
• Comply with applicable laws and healthcare provider instructions

12. No Guarantee of Medical Outcomes

Individual responses to medical treatment vary. DoctorAdam and participating healthcare providers do not guarantee any particular medical, therapeutic, cosmetic, weight-management, or other treatment outcome.

13. Intellectual Property

All website content, including text, graphics, branding, logos, interfaces, designs, software, and other materials, is owned by or licensed to DoctorAdam and protected by applicable intellectual property laws.

You may not reproduce, distribute, modify, publish, or commercially exploit DoctorAdam content without prior authorization.

14. Prohibited Uses

You may not use the Services to:

15.3 Class Action Waiver

To the fullest extent permitted by applicable law, disputes shall be brought on an individual basis and not as part of a class, consolidated, or representative action.

15.4 Waiver of Jury Trial

To the fullest extent permitted by law, you waive the right to a jury trial for disputes subject to valid and enforceable arbitration.

15.5 Exceptions

Nothing in this section prevents either party from seeking relief in small claims court where eligible or seeking temporary or preliminary injunctive relief where necessary to protect intellectual property, confidential information, security, or regulatory compliance.
