Tadalafil lowers blood pressure, so tell your provider about nitrates, alpha-blockers, and any heart condition before you start. An erection lasting more than four hours needs emergency care.

Tell your provider before you start if you take nitrates in any form, alpha-blockers for blood pressure or prostate symptoms, or medication for pulmonary hypertension — combined with tadalafil these can drop your blood pressure dangerously. The same applies if you have a heart condition, have had a stroke, or have been told sexual activity is not advisable for you. Your provider reviews all of this during the consultation.
